Complete remission of hepatic metastasis after total gastrectomy for a gastric carcinoid tumor type 1: a case report

Tresoldi Alberto Stefano , Bonifacio Cristina , Pepe Giovanna , Carnaghi Carlo , Lania Andrea Gerardo Antonio

Gastric carcinoids secondary to autoimmune atrophic gastritis (GC type 1) are usually well differentiated neoplasia, with an indolent course and an excellent overall prognosis. However, a subset of these tumors (<5%) may develop advanced disease, with lymph node and/or hepatic metastasis. The pathogenesis of these carcinoids is related to chronic trophic stimuli to enterochromaffin-like (ECL) cells due to chronic hypergastrinemia. Treatments directed to remove the source o...

Bilateral testicular masses and adrenal insufficiency in a young adult: is congenital adrenal hyperplasia the only possible diagnosis?

Tresoldi Alberto Stefano , Betella Nazarena , Pizzocaro Alessandro , Lania Andrea Gerardo Antonio

Testicular adrenal rest tumours (TARTs) are benign tumours deemed to originate from ectopic adrenal cells that descend with the testes during fetal development. These cells grow under chronic ACTH stimulation, typically in patients with congenital adrenal hyperplasia (CAH). TARTs have also been rarely described in other conditions characterised by chronically elevated ACTH, such as autoimmune primary adrenal insufficiency (PAI). These are benign lesions, but could be misdiagno...

Impact of body composition on skeletal health in subjects with Klinefelter’s syndrome: a cross sectional study

Carrone Flaminia , Vena Walter , Delbarba Andrea , Chiara Pezzaioli Letizia , Facondo Paolo , Cappelli Carlo , Pizzocaro Alessandro , Mazziotti Gherardo , Lania Andrea Gerardo , Ferlin Alberto

Background: There is growing awareness of skeletal impairment in individuals with Klinefelter syndrome (KS), involving both quantitative and qualitative alteration of the bone as well as an increased prevalence of vertebral fractures (VFs). Beyond hypogonadism, considerable evidence suggests that other factors may be responsible for the skeletal fragility observed in these patients. Abnormal body composition is a common finding in KS subjects, characterized by an unfavourable ...

Tresoldi Alberto Stefano , Toini Alessandra , Profka Eriselda , Locatelli Marco , Lasio Giovanni Battista , Morenghi Emanuela , Spada Anna , Mantovani Giovanna , Lania Andrea Gerardo

In this observational, retrospective, multicenter study, we analyzed data from 300 patients with pituitary incidentaloma followed in two Italian Hospital Center. We observed a predominance of female patients (65%), with an a mean age at diagnosis of 49 years (higher in men than women, 57 vs 45 years old). The main reason to perform imaging were neurological symptoms not related to the presence of adenomas (56%). Most cases were microadenomas (56%), and macroadenomas were more ...

Diagnostic features and surgical therapy of acromegalic patients: experience of the last three decades

Ferrante Emanuele , Sala Elisa , Locatelli Marco , Mantovani Giovanna , Malchiodi Elena , Carrabba Giorgio , Gaini Sergio Maria , Lania Andrea Gerardo , Spada Anna , Beck-Peccoz Paolo

Introduction: Generously supported by IPSEN)-->Acromegaly is a rare and insidious disease associated with an increased morbidity and mortality. Trans-sphenoidal (TNS) surgery remains the primary therapeutic option, in particular for intrasellar microadenomas and noninvasive macroadenomas. Aims of this study were to describe diagnostic features and to verify the impact of TNS surgery on treatment of acromegaly over three decades, befo...

GH response to oral glucose tolerance test: a comparison between patients with pituitary disease and healthy subjects

Verrua Elisa , Ronchi Cristina Lucia , Ferrari Daniela Ilaria , Olgiati Luca , Ferrante Emanuele , Filopanti Marcello , Lania Andrea Gerardo , Beck-Peccoz Paolo , Spada Anna

Background: GH response to oral glucose tolerance test (OGTT) is currently used for the definition of disease remission in acromegaly. This test has been poorly investigated in other pituitary diseases.Aim: To evaluate the impact of a pituitary disease other than acromegaly on GH response to OGTT.Patients and methods: Eighteen patients (13 F & 5 M, age: 50.7±13.1 years) with different pituitary diseases (i.e. non-functioni...

The aim of this study was to evaluate in a long follow-up time patients with acromegaly successfully treated by transphenoidal surgery (TNS) in order to establish the recurrence rate and the need of subsequent follow up.Methods: We retrospectively analyzed data of 283 acromegalic patients (168 females, mean age: 44.2 ±12.9 years) who underwent TNS for a GH secreting pituitary adenoma between 1980 and 2020, on regular follow-up at two Pituitary Units...

Background: Skeletal fragility is an important clinical issue in women with early-stage breast cancer and men with non-metastatic prostate cancer under hormonal hormone deprivation therapies (HDTs). Vertebral fractures (VFs) have been reported in a remarkable number of subjects exposed to HDT especially when the diagnosis of fractures is performed by a radiological and morphometric approach. Notably, prediction of fractures in this clinical setting is a challenge and determina...
